The School Function Assessment (SFA) is a test used to evaluate and monitor student’s performance of functional tasks and activities. The test can be used to help identify strengths and need of elementary school students with disabilities, in important nonacademic functional tasks. School personnel familiar with the student’s typical performance complete the SFA, evaluating performance on three different scales: Participation, Task Supports, and Activity Performance. Cut-off scores help establish eligibility for special services. The SFA complete kit Includes: 1 User’s Manual, 25 Record Forms, and 3 8-page Rating Scale Guides.
